Evaluates patient's previous living circumstances in context of current medical status to develop an appropriate discharge plan. Minimizes patient anxiety concerning illness and facilitates post- hospital referrals. Interacts with and supplements the health care team in order to enhance the social, emotional and psychological care of the patient/family/significant others. Willing and able to travel to multiple worksites. **Job Requirements** **Minimum Education Requirement:** MSW is required from an accredited school of Social Work. **Minimum Experience Requirement:** Minimum of 1 year experience working in a hospital environment is preferred. Excellent customer service skills required. **Minimum License Requirement:** BLS required. Delivering world-class health care one patient at a time.
